The Self-Study Modules on Tuberculosis 1-5 are currently
being revised. Continuing education credits are temporarily unavailable
for this course but will be provided with the revised modules.
Self-Study Modules on Tuberculosis 6 - 9
CME: CDC is accredited by the Accreditation Council
for Continuing Medical Education (ACCME) to provide continuing medical
education for physicians. CDC designates this educational activity
for a maximum of 16 hours in category 1
credit towards the AMA Physician's Recognition Award. Each physician
should claim only those hours of credit that he/she actually spent
in the educational activity.
CNE: This activity for 19.2 contact
hours is provided by CDC, which is accredited as a provider of continuing
education in nursing by the American Nurses Credentialing Center's
Commission on Accreditation.
CEU: CDC awards 1.6 Continuing
Education Units (CEUs). CDC is an Authorized CE Provider of the
International Association for Continuing Education and Training.
Registration
You can register for continuing education credits online or request
a print copy of the registration form and test for Self-Study
Modules on Tuberculosis 6-9.
Online Registration and Test
- Visit the CDC/ATSDR Training and Continuing Education Online
at http://www.phppo.cdc.gov/phtnonline/.
- Log on as a Participant or register as a New Participant to
create a user id and password.
- Once logged on to the CDC/ATSDR Training and Continuing
Education Online website, you will be on the Participant
Services page. Click on Search and Register.
In the Key Word type in SS3036 for modules 6-9.
- Immediately after you submit your registration and test online,
you will receive your test results.
Print Copy of Registration Form and Test
To request a print copy of the registration form and test, please
choose one of the following methods. For modules 6-9, request course
#SS3036.
- Email: Email DTBE at tbinfo@cdc.gov
and be sure to provide your name, address, daytime phone number,
and the course number.
- Fax: Place a toll-free call from a touch tone phone to
888-CDC-FAXX (888-232-3299). When prompted, enter document number
564007 to be sent to your 10- digit fax number. Complete the request
form and return it by fax to 404-639-8960.
- Phone: Call 404-639-8989 and ask for a continuing education
packet.
You should receive your CE registration packet in 2 to 3 weeks.
